What You’ll Do
- Perform comprehensive nursing assessments and develop individualized plans of care
- Provide skilled nursing care, patient education, and care coordination in the home
- Collaborate with physicians, therapists, medical social workers, and other members of the care team
- Monitor patient progress and communicate changes in condition promptly
- Complete accurate and timely documentation while maintaining Medicare and agency standards
Qualifications
- Current Florida Registered Nurse (RN) license
- Valid driver’s license and reliable transportation
- Excellent clinical, communication, and organizational skills
- Home health experience required
- OASIS experience required
