What You’ll Do
- Provide skilled nursing care in patients’ homes under the direction of the Registered Nurse and physician
- Perform ordered treatments, medication management, wound care, and patient education within the LPN scope of practice
- Observe, document, and report changes in patient condition to the supervising RN
- Collaborate with physicians, nurses, therapists, and other members of the care team
- Complete accurate and timely documentation while maintaining Medicare and agency standards
Qualifications
- Current Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license and reliable transportation
- Excellent clinical, communication, and organizational skills
- Home health experience preferred, but not required
- Experience with WellSky or another home health EMR is a plus
